Welcome to the AMR Kansas Uniform Apparel Order Form.
The following items are all approved uniform apparel with the most current AMR logo.
All items include first initial last name, with your title below. As well as the AMR Kanas Logo on the opposite side. You will be asked to enter your name and title on the form for each item.
All items are available in Navy Blue (field employees) and Black (supervisors) which you can indicate your preference on the order form.
You should get an email confirmation with tracking number when we ship your order.
Please be sure your order form is correct before submitting, due to the items being personalized we cannot accept exchanges or returns.
